I give permission to Humble Soccer Complex to photograph, video record, and use images of my child for promotional purpose, including but notlimited to social media, website, flyers and other marketing materials.

I understand that players are expected to demonstrate good sportsmanship, respect for coaches, referees, teammates, and opponents at all times. I agree to listen to my caoches, work hard, follow team rules, and treat everyone with respect. I understand that inappropriate behavior my result in disciplinary action, including removal form the program without a refund.

I agree to support all players, coaches, referees and league officials in a positive manner. I will encourage good sportsmanship, set a good example, and refrain from negative or disrespectful language or actions at practices and games. I unerstand that my child's experience is more important than the outcome of the game.

I herby certify that my child is physically able to participate in soccer activities. I understand that soccer involves physical contact and there is a risk of injury. I release and hold harmless Humble Soccer Complex, its coaches, volunteers, staff and affiliates from any and all liability for injuries or illineses that may occur during participation.
